Referrer Information

Complete the Referrer Information screen about yourself. 

1.  Click on Start next to Referrer Information on the left side.  

2.  Select your Relationship to the child from the drop-down list.  Click the Save and Continue button.

3.  Type Contact phone number.

4.  Click the Save and Continue button.

5.  The Child Information screen will appear.
